Thank you for booking a place at Baslow Junior Football Festival 2018. There are opportunities for U7, U8, U9 and U10 teams of boys, girls and/or mixed teams. U7 and U8 teams will consist of five players plus two substitutes (7 players max) and U9 and U10 teams with seven players plus two substitutes (9 players max).
Please do not exceed the maximum number of players for each team and if you submit two teams of the same age group, please note that players are not interchangeable between teams.

NB age groups are determined as per the school year so for this festival, the age groups are for the 2017/18 season.

Personal accident insurance is mandatory for each team.

Please note that booking fees are non refundable. In the unfortunate event that we do not receive sufficient bookings to make a group viable, a full refund will be paid.

Bookings are only confirmed on receipt of payment.

This is a very popular event so bookings will be closed when each age category (league) is fully subscribed.

This event is sanctioned by Derbyshire County FA. Players, teams, their affiliates and officials are subject to the disciplinary processes of the County Football Association. Please list each team separately. All boxes should be completed.

You will need to make a new booking for each team you register. Email your details from the email link below and then pay online by registering your team below. To pay once you have sent your email click on the relevant link below for the team you wish to register and follow the instructions. If you are a member of Baslow Sports Field just log in when asked. If you are not a member you will be asked to create an account. This is free and you must enter your details when prompted. Once signed up register your players. Do not worry if you do not know all of their names and details at this stage, this can be completed at any time up to the event. Complete your contact details in the relevant boxes and enter your affiliation number in the box marked British Tennis Number. When finalising your booking you will be asked if you are applying on behalf of an organisation. Please tick the box and enter the name of your team.
