STAGE 2: MEMBERSHIP NEXT STEPS (10 minutes) - ONE FORM PER MEMBER

This form must be completed to comply with LTA regulations and applies to all adults and juniors. It also contains important imformation about policies and the Club.

Family memberships - an adult must complete a Statement for Acceptable Use of Internet and Social Media FOR EACH CHILD.
Once fees and all relevant form(s) are received, a shoe tag will be issued and email will be sent to confirm your membership and ClubSpark account is active.
