By signing up to a coaching course you are agreeing to the following-

 I agree to my child participating in the course. I understand that the club coaches have a third party policy, which provides cover to my child against injury, providing that our club, its servants or agents, have been negligent. I understand that parents may take out their additional insurance cover if they so wish. 

If in the event of an emergency and  you cannot be contacted on the provided emergency contact numbers you are giving your consent for your child to receive medical attention.
